Default Custom dbSuggestionForm - multiple columns

The help for dbSuggestionForm says multiple columns are supported but near as I can tell, little else is said about that feature. I remember when the control first came out there were some efforts, with mixed results, to create a custom multiple column package but the control has evolved since then. Before I go off and try to create such, has anyone successfully gotten this to work and can share an example? My interest is with a database and not static data.

Update - Oops, I guess it hasn't changed, bad assumption. The mixin package hasn't changed since 2014. I guess "multiple columns" must be referring to something else?
